Accessing Your Medical Record

All patients are entitled to access their medical record held by their GP. There are 3 options:

  1. Register for online access to your medical record.
  2. Request a copy of your full medical record
  3. Request a part copy of your medical record

Request a copy of your Medical Record

This is called a Subject Access Request and we prefer patients to make the request in writing to:

Please provide

  1. Your full name, address, date of birth
  2. Full details of your requirements. If just a part of the record then please specify the date range.


What happens next

  • A single copy of your medical record will be provided free of charge. There may be a reasonable administration charge for multiple requests in accordance with General Data Protection Regulation.
  • We will endeavour to have the record prepared within 21 days but a maximum of 28 days.
  • You will be asked to provide proof of your identity before the record is released.
