Welcome to our practice, formed on October 1st 2014, by the merger of North End, Steeple Claydon and Masonic House Surgeries. Verney Close Surgery joined with us on 1st July 2016.

We are confident that our website will provide clear and concise help and give our patients the information they require in an easy and convenient format. It has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us.

 Practice Welcomes New GPs

"Hello I'm Dr Hadi.
A relatively local boy, I grew up in Milton Keynes and attended Aylesbury Grammar School. I attained my medical degree from King's College London and have recently qualified as a GP under the Oxford deanery. My final year of training has been at the Swan Practice, and I'm delighted to have been given the opportunity to continue working here.
In my spare time I enjoy motorcycling, flying, football and travelling; with as much music mixed in there as possible!"

I am Dr Claire Chappell and I am delighted to join The Swan Practice.

I completed my medical degree at The University of Leicester in 2011. Since then I have worked in various departments at The John Radcliffe Hospital in Oxford, High Wycombe Hospital and Stoke Mandeville Hospital.

During my time working as a hospital doctor I decided I wanted to be a GP, as I enjoy building relationships with patients and providing continuity of care. I’ve not regretted this choice and continue to enjoy the variety General Practice brings. I have been qualified as a GP for almost three years now and I am interested in women’s health. I have undertaken further training in women’s health covering family planning, contraceptive services including the fitting of contraceptive coils.

Outside of work I can be found either in the gym on a spinning bike, in the kitchen baking or outdoors in the beautiful Buckinghamshire countryside.

NHS Primary Care Networks

Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease, or suffer with mental health issues and may need to access their local health services more often.
To meet these needs, practices have begun working together and with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in primary care networks.

Primary care networks build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care.
Primary care networks will be based on GP registered lists, typically serving natural communities.  They should be small enough to provide the personal care valued by both patients and GPs, but large enough to have impact and economies of scale through better collaboration between practices and others in the local health and social care system.

The Swan Practice is part of the North Bucks Primary Care Network, who aim to provide the best possible care for patients in the north Bucks area.
